This is an intake form only. Submission does not guarantee permit approval. Depending on the event, venue, and whether the event is open to the public, additional approvals or documentation may still be required.

If your event is within five days, you must submit your application to your local district office.

An event that ends by 2:00 AM is considered a single day for purposes of your application dates and fees.
